The Rail Accident Investigation Branch has made 21 recommendations on how Network Rail should improve safety systems in its final report into the fatal Grayrigg crash.
The recommendations cover improvements to the company’s risk management systems, improvements to track inspection, maintenance and design procedures and the need for research into the link between long work hours and human error.
